Here’s the FMP’s sharpest edge: the program cannot issue preauthorizations. Care abroad happens first; adjudication happens after. For a $150 follow-up visit, that’s a shrug. For surgery, it’s a five-figure question mark — unless you use the tool VA built for exactly this: the treatment predetermination screening.
What a predetermination is (and isn’t)
Through the Ask VA system, you can communicate directly with the FMP office and request a treatment predetermination screening for care you’re planning. Per VA’s own provider guidance, this process provides reasonable assurance on whether a claim will be paid, if billed appropriately.
Note both halves of that sentence. It’s reasonable assurance, not a contractual guarantee — and it’s conditioned on proper billing. A predetermination won’t save a claim whose paperwork fails to tie treatment to your rated condition. It answers the coverage question in advance; the documentation still has to hold up.
How to request one that gets a useful answer
Vague questions get vague answers. Give the FMP office the same specifics an adjudicator would need later:
- Identify yourself as FMP-registered (register first — here’s how) and reference the service-connected condition from your benefits authorization letter, exactly as listed.
- Name the specific treatment. “Total knee arthroplasty, right knee” beats “knee surgery.” Include the planned country and approximate timeframe.
- Connect the dots in one sentence. The treatment is medically necessary for the rated condition — say so plainly, and mention any supporting clinical records you hold (imaging, specialist notes, VA treatment history for that condition).
- Ask the payment-shaped question: would a properly documented and billed claim for this treatment, for this rated condition, be payable under FMP?
Timing the trip around the answer
Build your sequence so money commits last: predetermination request → response in hand → surgeon consult scheduled (many Colombian clinics will do records-based or video pre-consults) → then flights and deposits. If the predetermination comes back unfavorable, you’ve lost nothing but a little time — and you’ve learned whether the gap is coverage itself or just how the request was framed.
Emergencies are the exception, not the plan
If you’re abroad and an emergency involving your service-connected condition hits, get care immediately at the nearest capable facility — FMP covers emergency and urgent care tied to covered conditions, and reimbursement documentation comes after. The predetermination pathway is for planned care, where you have the luxury of asking first. Use the luxury.
