Most veterans who’ve heard of the FMP picture it one way: pay a foreign hospital out of pocket, then chase VA for reimbursement. That’s one mode. The other — buried in VA’s own guidance — is that your provider may file the claim on your behalf, and VA pays the provider directly. Done right, you never front the surgical bill at all.
The two payment modes, side by side
| Provider files (direct billing) | You file (reimbursement) | |
|---|---|---|
| Who pays first | Nobody — VA pays the provider | You pay the provider up front |
| Your cash exposure | Minimal for covered care | Full cost until the claim pays |
| Who assembles paperwork | The clinic (you still keep copies) | You |
| Best for | Planned surgery, larger bills | Small visits, clinics unwilling to file |
Either way, the coverage rules are identical: the care must be medically necessary for a service-connected condition, documented as such, and billed appropriately. Direct billing changes cash flow, not eligibility.
Why every foreign clinic doesn’t do this already
Because filing with a U.S. federal program is unfamiliar administrative work, and VA pays after adjudication rather than at the front desk. A clinic that has never seen an FMP claim has no reason to trust the process — unless someone walks them through it, shows them the documentation standard, and demonstrates that properly filed claims for clearly covered care get paid.
That someone has to be local, bilingual, and patient. This is precisely the groundwork we do in Medellín: sitting with clinic administrators, explaining the FMP’s requirements (itemized billing, notes tying treatment to the rated condition, the veteran’s authorization letter), and building the direct-bill relationship before a veteran is on the table.
How to set it up for your own care
- Register and get your predetermination first. A clinic is far more willing to bill VA when you arrive with a benefits authorization letter and a favorable predetermination response in hand — it de-risks the arrangement for them.
- Raise it at the consult, not at checkout. Ask directly: will you file the FMP claim and accept VA payment? Get the answer in writing along with the treatment plan.
- Agree on the fallback. If the clinic won’t file, know your number: what you’d pay up front and claim back. Typical 2026 private-pay ranges in Colombia often make even the reimbursement path workable — but know before you go.
- Keep parallel copies of everything. Provider filing doesn’t outsource your interest in the paperwork. Your copies are the backstop, per our documentation checklist.
The bigger picture for veterans in the queue
Direct billing is what turns the FMP from a reimbursement lottery into an actual care pathway: a veteran with a service-connected knee, a 100-day VA orthopedic backlog at home, and a predetermination in hand can be treated at a JCI-accredited Colombian hospital by a ReTHUS-verified surgeon — with VA paying the hospital. That’s not a loophole. It’s the program working as written in 38 CFR § 17.35, used by someone who read the manual.

Send us your rated condition and what you’re waiting on. We’ll give you an honest read on FMP fit, the predetermination step, and which Medellín providers make sense — and if the answer is “this won’t be covered,” we’ll say exactly that. Operated in person from Medellín via Colombia Medical.
Check my FMP fitFrequently asked questions
Is direct billing officially allowed under the FMP?
Yes. VA's own guidance states your health care provider may file your FMP claim on your behalf, and VA pays the provider directly. If the provider doesn't file, you pay and claim reimbursement.
Will VA pay the clinic before my treatment?
No. FMP has no preauthorization mechanism; claims are adjudicated after care. Direct billing means the provider waits for VA payment instead of you paying up front - which is why clinics want to see your authorization letter and predetermination first.
What costs does direct billing NOT cover?
Anything outside FMP coverage: travel, lodging, companion expenses, and any care not tied to your service-connected condition. Those are always out of pocket.
Can you introduce me to Medellin clinics that will direct-bill?
That's the groundwork we do locally - building FMP-literate relationships with credentialed providers. Contact us with your rated condition and the care you need, and we'll tell you honestly what's available.
